Official Troop 843 Forms and Policies
- BSA Annual Health and Medical Form
- This Annual Health and Medical Record replaced the Class 1, 2 and 3 Medical forms, and the Medication
Card. This form will become the standard in January of 2010, and should be used from now on. You may download
this form to your computer, edit in your information, and then save it somewhere on your computer. You can then
simply print it out, with all your information already in place, whenever you need a copy.
